Led by Dr. Madan Mohan, the cardiac team treated a 75 year old woman with severe aortic stenosis and AV block through TAVI and dual chamber leadless pacemaker implantation, allowing discharge within 48 hours
MGM Healthcare Malar, Adyar has performed India’s first combined catheter based cardiac intervention involving Transcatheter Aortic Valve Implantation and dual chamber leadless pacemaker implantation in a single patient, avoiding open heart surgery.
The patient, a 75 year old woman with a history of bypass surgery, was admitted with severe breathlessness that affected her daily life. She had also experienced three episodes of loss of consciousness over three months, pointing to a serious cardiac condition.
A detailed evaluation, including CT Aortogram, showed severe aortic stenosis, where the heart’s main valve becomes critically narrowed. Doctors also identified a 2:1 atrioventricular block, which disrupted the heart’s electrical conduction. Her age, previous cardiac surgery, and combined valve and rhythm problems made her a high risk case.
The multidisciplinary cardiac team, led by Dr. Madan Mohan, Senior Consultant, Interventional Cardiologist, chose a fully percutaneous treatment approach instead of conventional open heart surgery.
The team first performed TAVI to replace the diseased aortic valve. This was followed by implantation of a dual chamber leadless pacemaker system, in which two miniature devices were placed directly inside the heart chambers. Unlike traditional pacemakers, this system does not require surgical pockets or lead wires and allows wireless communication between devices to regulate heart rhythm.
Both procedures were performed through a blood vessel in the leg. This helped avoid open heart surgery, reduce infection risk, limit intensive care needs, and support faster recovery. The patient was discharged within 48 hours of the intervention.

This combined approach represents a major advancement in structural and rhythm management therapy and highlights the growing shift toward fully catheter based, device driven cardiac care for high risk patients.”
