February 17, 2026: MGM Healthcare Malar Adyar has performed what it describes as the city’s first multi level endoscopic Transforaminal Lumbar Interbody Fusion procedure on a 75 year old patient suffering from severe lower back pain and restricted mobility caused by nerve compression and age related spinal degeneration.
The surgery involved fusion at two lumbar levels, L3 L4 and L4 L5. The minimally invasive procedure relieved nerve pressure and stabilised the spine by permanently joining the affected vertebrae. The patient was mobilised within hours of surgery and discharged the following day.
Spinal fusion is performed when a segment of the spine becomes unstable or continues to cause pain despite conservative management such as medication, physiotherapy or injections. By eliminating movement at the affected segment, the procedure reduces mechanical pain and prevents ongoing nerve compression.
Endoscopic Transforaminal Lumbar Interbody Fusion is carried out through small incisions using specialised instruments. Compared to conventional open surgery, the approach limits muscle disruption, reduces blood loss and lowers the risk of infection, particularly in elderly patients.
The surgical team was led by Dr Vignesh Pushparaj, Senior Consultant Orthopaedic Spine Surgeon, MGM Healthcare Malar Adyar, India.
Dr Vignesh Pushparaj said, “The patient presented with severe lower back pain and significant restriction of movement. Imaging confirmed the need for fusion at two levels. Instead of conventional open surgery, we chose a minimally invasive approach. The patient was mobilised on the same day, walked independently and was discharged within 24 hours.”
He added that traditional open spine surgery involves larger incisions and muscle retraction, which can increase tissue trauma and delay recovery. The endoscopic approach reduces surgical morbidity and supports faster rehabilitation in senior patients.

The medical team also advised maintaining correct posture, avoiding prolonged sitting beyond 90 minutes without breaks, and ensuring regular movement to support long term spinal health.
